Protocol has become a lot more relaxed in the last several decades, if someone were to call her Elizabeth, then that person is more than likely to be a Republican (they want a democratically elected head of state and no bloomin Royal Family sucking money off the taxpayer) or the Duke Of Edinburgh in one of his many gaffe moments. I think i need to make things clearer - Most British people have a lot of respect for the Queen, though that cannot be said about Charles. Which is probably why the Queen has taken Prince William under her wing by all accounts. He is the future of the Royal Family.
There have been a lot of breeches in protocall over the last 20 years, the Royal Family no longer commands the respect of the nation, apart from the Queen (though she was in dangerous waters after Diana's death).
I don't see Barak Obama being elitest by calling John Mccain by his first name. I think that's just making a mountain out of a molehill, to prove an already formed view of the man.
